Individual Health Plans and Emergency Action Plans

Each Individual Health Plan is developed by a registered nurse with your student's health, access to education and activities as the primary purposes. Health Plans are not IEP/504 plans; however, they may be an addition to one of those plans or as a stand-alone document. These plans must be developed or renewed each year at the beginning of the year. Each health plan is uniquely positioned to inform educational teachers and staff of individual student health needs. Goals and outcomes are designed to help your student have equal access to all educational opportunities. Emergency Action plans are designed to provide information for school staff in the event of an emergency related to a specific diagnosis. Both of these plans are developed in a collaborative atmosphere with student/parents/physician/registered nurse/educational staff. Emergency Action Plans are required for the following diagnoses:

  • Asthma
  • Allergy that requires epi-pens
  • Self-managed diabetes students
  • Seizures
