Medication Administration Information

There are a few forms that you need to fill out and may need to have signed by the physician before medications can be administered at school. Here is the list:

General Medication Form and Consent

  • Medication Consent Form
  • Prescription medications given at school.
  • OTC medications (i.e., acetaminophen, ibuprofen, ointments, etc.) that do not require a physician's signature.
  • Emergency Action Plans for epi-pens, seizure meds, diabetic rescue meds, asthma inhalers are also required to have the medication consent form for these emergent situations and signed by the provider.

IMPORTANT NOTE:

Medication must be brought to the school office by the parent/guardian with appropriate forms and signatures. Medication must be:

  • In the original bottle and with label attached.
  • Must not be expired
  • Medication for self-carry students must be checked in with the District Nurse to confirm accuracy with a signed self-administer agreement and provider order.
