All medications at school whether self-carry inhalers, emergency medications, diabetes, seizure, or OTC medications must have a consent form and self-carry agreement (if applicable). These forms are available under the medication forms tab. Please plan ahead and get these signed over the summer. They are due by the first day of school. No medication will be given by nurse or trained school staff without these consents.
