Medication Administration Information
There are a few forms that you need to fill out and may need to have signed by the physician before medications can be administered at school. Here is the list:
General Medication Form and Consent
- Medication Consent Form
- Prescription medications given at school.
- OTC medications (i.e., acetaminophen, ibuprofen, ointments, etc.) that do not require a physician's signature.
- Emergency Action Plans for epi-pens, seizure meds, diabetic rescue meds, asthma inhalers are also required to have the medication consent form for these emergent situations and signed by the provider.
IMPORTANT NOTE:
Medication must be brought to the school office by the parent/guardian with appropriate forms and signatures. Medication must be:
- In the original bottle and with label attached.
- Must not be expired
- Medication for self-carry students must be checked in with the District Nurse to confirm accuracy with a signed self-administer agreement and provider order.
All forms are available under the new school year tab. Please contact Amy Jensen, RN for questions or concerns. 208-851-9953 (district cell phone), amy.jensen@psd201.org
